Diabetes Management
Individuals who may benefit from this referral include those recently diagnosed with diabetes or anyone who has previously been diagnosed and whose blood glucose is not within goal. We provide diabetes treatment and education. We will assist patients with insulin pumps, glucometers and managing medications. In addition, we will teach patients how to be active, eat healthy, monitor blood glucose and administer medications. Visits are covered by most insurances.
